## Abstract

Clinician commentary on COXEN in the sources reviewed for this edition leads to one open question: What evidence and combined clinical assessment would justify adding, withholding or curtailing treatment without overstating a biomarker's clinical utility? Its leading findings: (1) A response biomarker is not, by itself, permission to omit cystectomy. (2) GC versus ddMVAC remains a conditional choice, not a settled equivalence or universal superiority claim. (3) adding perioperative IO versus reserving risk-adapted adjuvant IO remains a live sequencing dispute. 7 more findings follow. 203 sources screened, 66 cited; not all HCPs or all public discussion.

## Open Medical Affairs question

What evidence and combined clinical assessment would justify adding, withholding or curtailing treatment without overstating a biomarker's clinical utility?

Raised by the leading finding. The report shows what clinicians said and the sources behind it; it does not settle the question.

## Findings (titles)

- F1. A response biomarker is not, by itself, permission to omit cystectomy
- F2. GC versus ddMVAC remains a conditional choice, not a settled equivalence or universal superiority claim
- F3. Adding perioperative IO versus reserving risk-adapted adjuvant IO remains a live sequencing dispute
- F4. ctDNA is described as strongly prognostic, but not yet a general rule for starting or stopping IO
- F5. Favorable prognosis after pCR is not the same as trial-level surrogacy for survival benefit
- F6. Failure of the original treatment-specific COXEN objective did not erase the perceived value of S1314
- F7. Multimodal AI attracts strong enthusiasm, with clinical utility and external validation still unresolved
- F8. cfDNA methylation is presented as exploratory proof of concept, not a validated reason to withhold chemotherapy
- F9. Prior BCG and secondary MIBC generate biological questions, not a current mandate to withhold cisplatin-based NAC
- F10. Referral, treatment delivery and local-treatment burden are part of the clinical question

## What sits behind each finding

Counts describe what the report retained, not how many clinicians hold a view. A statement can support more than one finding.

## Scope and limits

This report does not judge efficacy, estimate HCP prevalence or consensus or give treatment advice.

- Discussion that does not name the trial may be missing from this edition.
- 1 cited commentary passage leaves a source question open, such as who was speaking. The report attributes no further than the source shows.
- Not yet captured in this edition: a view on survival evidence informing neoadjuvant versus adjuvant immunotherapy alongside chemotherapy and surgery.
- Some passages in the sources used in this report were not fully assessed for clinician views, so a view there may be missing from this edition.
- 320 statements in the sources reviewed for this edition are about other patients or topics, or only mention the trial, and are not counted.
- Further source limits apply, so some matching discussion may be missing.
- The sources reviewed for this edition do not represent all HCP discussion or support estimates of professional consensus. Multiple posts, reposts or sources are not necessarily independent assessments.
- Trial-name and identifier-focused discovery may miss discussions using unnamed references or different transcription variants. Some available passages were not fully assessed for clinician views, so absence from an aspect is not proof that the issue was never discussed.
- Some spoken passages cannot be assigned to a named participant. They may belong to a named participant in the same program and must not be counted as additional voices.
- A relay preserves another party's statement; it does not establish that the relayer independently endorses the interpretation. Research involvement, professional identity or an organizational account does not increase evidentiary weight.
- The discussions span different evidence maturities, populations and treatment-access circumstances. Approval, reimbursement, investigational status and anticipated readouts are time-specific, not a current comprehensive regulatory assessment.
- S1314 was not designed to settle all secondary regimen or response–survival comparisons, and reduced biomarker evaluability limits precision. Nonsignificance is not equivalence, and prognostic association is not proof of treatment-selection benefit.
- Gene-panel descriptions, unusual regimen wording and methylation performance percentages are not fully consistent across sources. The report preserves those differences rather than selecting an unverified correction.
- Unclear wording about patients not proceeding to surgery does not establish progression, toxicity or another specific clinical reason.
