## Abstract

DESTINATION 2 asks whether lowering radiotherapy dose to benign-appearing prostate while preserving dose to MRI-visible tumours can reduce urinary toxicity without compromising cancer control in localized prostate cancer. Commentary comes from X threads, podcasts and a YouTube video. The discussion turns on whether the two-fraction dose is adequate, how to interpret contested target-specific margin reduction, and what conditions would make MR-guided adaptation scalable.

## Open Medical Affairs question

What evidence of preserved cancer control, meaningful patient-reported benefit and reproducible delivery would justify moving this experimental approach beyond its current research setting?

Raised by the leading finding. The report shows what clinicians said and the sources behind it; it does not settle the question.

## Findings (titles)

- F1. Support for de-escalation does not settle whether the two-fraction dose is sufficient
- F2. The margin controversy is target-specific and includes biological as well as geometric uncertainty
- F3. Whether focal escalation adds benefit depends on the whole-gland dose, fractionation and evidence standard
- F4. Selection includes surveillance and the option of a longer course, not just choosing a boost
- F5. MRI-enabled targeting is attractive, but lesion definition and response adaptation remain uncertain
- F6. Urethral protection ranges from selective contouring to routine sparing
- F7. Rectal safety concerns are expressed through concrete planning safeguards and reluctance to escalate further
- F8. Spacer use remains a conditional benefit–burden discussion rather than a settled requirement
- F9. The early-toxicity endpoint is defended as developmental evidence, not a substitute for mature outcomes
- F10. Federated recruitment is welcomed, but operational success and statistical interpretation remain separate
- F11. Scalability depends on staffing, contour consistency and demonstrating the value of MR-guided adaptation
- F12. Research incentives and reimbursement can obstruct otherwise attractive treatment-shortening research

## What sits behind each finding

Counts describe what the report retained, not how many clinicians hold a view. A statement can support more than one finding.

## Sample findings

Paraphrased; the full report links each statement to its source.

### F1. Support for de-escalation does not settle whether the two-fraction dose is sufficient

An X thread asked how to assess dose adequacy for the two-fraction prostate radiotherapy tested in DESTINATION 2.

The sources reviewed for this edition do not confirm the author's clinical role.

Source: X thread, 2026-05-08.

### F9. The early-toxicity endpoint is defended as developmental evidence, not a substitute for mature outcomes

A podcast discussion asked how early toxicity assessments relate to clinically important longer-term outcomes.

The relevant podcast passage does not identify who was speaking.

The sources reviewed for this edition provide no DESTINATION 2 clinical outcome results.

## Scope and limits

This report does not judge efficacy, estimate HCP prevalence or consensus or give treatment advice.

- Discussion that does not name the trial may be missing from this edition.
- Not yet captured in this edition: a view on imaging to avoid missing cancer when selecting tissue for prostate radiotherapy.
- Not yet captured in this edition: a remark on trial origins, hypothesis-testing role, protocol availability, ongoing recruitment and feedback shaping experimental design.
- Some passages in the sources used in this report were not fully assessed for clinician views, so a view there may be missing from this edition.
- 57 statements in the sources reviewed for this edition are about other patients or topics, or only mention the trial, and are not counted.
- Further source limits: discussion that describes the trial without naming it may be incomplete; some discussion that describes the trial without naming it may be missing; sources about other trials are not included.
- The report covers only the sources reviewed for this edition, not all clinicians or all public discussion. Some potentially relevant passages were not fully assessed, and trial-name searches may miss unnamed references or wording variants.
- No DESTINATION 2 clinical outcome results are provided. The protocol publication describes planned methods and hypotheses, while interview and social-media recruitment statements describe progress at their own timepoints.
- Several podcast passages cannot be assigned to a named speaker. They are not attributed to a speaker simply because the same episode includes clearly attributed statements from the speaker.
- Some speakers' identities or clinical roles are not established in the sources used in this report; each statement is attributed only as far as its source shows.
- Trial involvement and presentation roles are described as the sources describe them; such involvement does not make a view independent external validation.
- External-study comparisons, radiobiological calculations, economic arguments and personal experience remain attributed interpretations. They do not independently establish DESTINATION 2 efficacy, toxicity, procedural safety or cost-effectiveness.
- Views about other fractionations, brachytherapy and planning safeguards are included where they inform the localized-prostate treatment decision. They are not presented as DESTINATION 2 protocol requirements.
