## Abstract

DURGA-6 compares AZD0120 CAR-T consolidation with autologous stem cell transplantation in transplant-eligible patients with newly diagnosed multiple myeloma. Commentary comes from X posts, a podcast and YouTube discussions featuring physicians, including medical oncologists, and a hematology–oncology nurse practitioner. Debate centers on whether CAR-T will displace transplantation, an unsettled prospect, and how sustained MRD negativity should be interpreted. Another question is whether rapid manufacturing offers biological advantages, which remain hypothetical in these discussions.

## Open Medical Affairs question

What evidence and support would make CAR-T a replacement for ASCT rather than another individualized consolidation option?

Raised by the leading finding. The report shows what clinicians said and the sources behind it; it does not settle the question.

## Findings (titles)

- F1. ASCT displacement is a forecast, not a settled replacement strategy
- F2. Earlier Chinese frontline findings generate enthusiasm, but are not DURGA-6 outcomes
- F3. Sustained MRD negativity is the interpretive priority, not a single negative sample
- F4. Rapid manufacturing and dual targeting attract interest, while biological advantages remain hypotheses
- F5. Eligibility and access depend on fitness, manufacturing time, preference and early collaboration
- F6. The manufacturing interval remains a treatment-sequencing problem
- F7. A single infusion and outpatient delivery do not eliminate continuing care
- F8. Early toxicity treatment is described as evolving institutional practice, not an AZD0120 protocol

## What sits behind each finding

Counts describe what the report retained, not how many clinicians hold a view. A statement can support more than one finding.

## Sample findings

Paraphrased; the full report links each statement to its source.

### F3. Sustained MRD negativity is the interpretive priority, not a single negative sample

In a YouTube discussion, a medical oncologist asked how sustained MRD negativity should be interpreted in the earlier Chinese frontline study.

### F4. Rapid manufacturing and dual targeting attract interest, while biological advantages remain hypotheses

In the YouTube discussion, the same medical oncologist asked whether FasT-CAR manufacturing could improve efficacy in addition to shortening production time.

## Scope and limits

This report does not judge efficacy, estimate HCP prevalence or consensus or give treatment advice.

- Discussion that does not name the trial may be missing from this edition.
- Some clinician views in the sources used in this report are not yet captured as evidence in this edition.
- Some passages in the sources used in this report were not fully assessed for clinician views, so a view there may be missing from this edition.
- 76 statements in the sources reviewed for this edition are about other patients or topics, or only mention the trial, and are not counted.
- Further source limits: discussion that describes the trial without naming it may be incomplete; some discussion that describes the trial without naming it may be missing; sources about other trials are not included.
- The sources reviewed for this edition provide perspectives from a small set of conversations, not a representative measure of HCP opinion. Podcast and video versions of the ONS session must not be counted as independent support.
- No publication of results for NCT07735637 was found, and no protocol was available among the sources reviewed. The report cannot establish comparative efficacy, trial-specific safety or detailed protocol requirements.
- Earlier Chinese frontline findings include both transplant-eligible and transplant-ineligible patients. Their favorable interpretation does not establish benefit over ASCT in DURGA-6.
- Several treatment-choice statements do not name DURGA-6. They are contextual views on the relevant clinical decision, not direct evaluations of the trial's outcomes or conduct.
- Some podcast remarks about future treatment roles do not identify who spoke. Those passages remain unnamed and are not treated as additional independent views.
- Discussion using unnamed trial references, alternative terminology or different transcriptions may be missing, and some additional X material was not fully assessed.
- No exact AZD0120 product label was located, and the broader regulatory review was incomplete. General CAR-T practices in these discussions cannot be treated as AZD0120 prescribing instructions.
- The safety discussion is selective: the institutional policy on prophylactic tocilizumab and concerns about prolonged cytopenias, clonal marrow changes and secondary malignancies are not characterized here. This limits conclusions about preventive care and long-term safety.
- Not yet captured in this edition: a view on geographic CAR T access barriers and developing CAR T capacity at transplant centers.
