## Abstract

Clinician commentary on EPCORE DLBCL-2 in the sources reviewed for this edition leads to one open question: What absolute benefit, survival, safety, subgroup and delivery evidence would justify adding treatment rather than continuing an established frontline option? Its leading findings: (1) the central disagreement is what PFS improvement must prove before practice changes. (2) numerical safety and deaths—not qualitative reassurance—are pivotal to interpretation. (3) the actual treatment choice remains unresolved: R-CHOP, Pola-R-CHP or epcoritamab plus R-CHOP. 9 more findings follow. 83 sources screened, 55 cited; not all HCPs or all public discussion.

## Open Medical Affairs question

What absolute benefit, survival, safety, subgroup and delivery evidence would justify adding treatment rather than continuing an established frontline option?

Raised by the leading finding. The report shows what clinicians said and the sources behind it; it does not settle the question.

## Findings (titles)

- F1. The central disagreement is what PFS improvement must prove before practice changes
- F2. Numerical safety and deaths—not qualitative reassurance—are pivotal to interpretation
- F3. The actual treatment choice remains unresolved: R-CHOP, Pola-R-CHP or epcoritamab plus R-CHOP
- F4. Some reactions are emphatic, but most practice-change language remains prospective
- F5. Risk selection is central; benefit should not be assumed uniform across eligible patients
- F6. EPCORE NHL-2 supplied the rationale, not a randomized estimate of frontline benefit
- F7. Interim timing, independent review and unequal randomization shape confidence in the signal
- F8. Duration and scheduling raise questions about additional treatment burden and trial design
- F9. Access, capacity and cost could determine whether a promising regimen is deliverable
- F10. Molecular response is proposed as a route to selective escalation—and eventually less treatment
- F11. Open sequencing questions coexist with predictions of a sharp decline in cellular therapy
- F12. Regulatory fit and the competing evidence base remain conditional

## What sits behind each finding

Counts describe what the report retained, not how many clinicians hold a view. A statement can support more than one finding.

## Scope and limits

This report does not judge efficacy, estimate HCP prevalence or consensus or give treatment advice.

- Discussion that does not name the trial may be missing from this edition.
- Not yet captured in this edition: a view on alternative bispecific-antibody chemotherapy strategies for previously untreated intermediate- or high-risk diffuse large B-cell lymphoma.
- Not yet captured in this edition: a view on biology-guided selection of targeted additions to frontline treatment in newly diagnosed diffuse large B-cell lymphoma.
- Not yet captured in this edition: a view on frontline CAR-T as an alternative to chemoimmunotherapy escalation in very-high-risk newly diagnosed lymphoma.
- Not yet captured in this edition: a view on targeted additions to frontline chemotherapy for high-risk newly diagnosed diffuse large B-cell lymphoma.
- Not yet captured in this edition: a view on frontline epcoritamab plus R-CHOP efficacy, safety and the need for randomized evidence.
- Not yet captured in this edition: a view on frontline bispecific escalation using an alternative chemoimmunotherapy backbone in newly diagnosed diffuse large B-cell lymphoma.
- Some passages in the sources used in this report were not fully assessed for clinician views, so a view there may be missing from this edition.
- 75 statements in the sources reviewed for this edition are about other patients or topics, or only mention the trial, and are not counted.
- Further source limits: discussion that describes the trial without naming it may be incomplete; some discussion that describes the trial without naming it may be missing; sources about other trials are not included.
- The sources reviewed for this edition do not represent all HCPs or all public discussion. Some relevant passages were not fully assessed, and unnamed or differently worded references may be missing.
- Source totals do not measure independent HCP support: the material includes repeated posts, quoted announcements, organizational accounts and reactions to the same information.
- No original results publication or full protocol for EPCORE DLBCL-2 was available. Phase III result descriptions are largely sponsor topline information relayed through commentary, and some methods are secondary descriptions.
- At the time of the cited topline discussions, absolute PFS by arm, OS, detailed response and safety outcomes, follow-up, event counts and geographic enrollment were not fully reported. Their absence cannot establish no benefit, no harm or lack of applicability.
- Earlier EPCORE NHL-2 findings concern a small high-risk cohort, different treatment exposure and different reporting time points. They cannot be pooled with, or substituted for, randomized EPCORE DLBCL-2 outcomes.
- Some speakers' identities or clinical roles are not established in the sources used in this report; each statement is attributed only as far as its source shows.
- This edition does not fully capture the February 2026 reasons for selecting or withholding alternative frontline regimens, or every conditional question about subsequent cellular therapy. It should not be used as a complete account of those exchanges.
- Predictions about standard of care, cure, CAR-T displacement, later-line research funding, regulatory pathways and timing remain predictions. References to other settings are used only where the speaker explicitly applies them to interpreting this frontline decision.
- Post and publication dates are not necessarily session dates. Pre-result expectations, earlier-study interviews and topline reactions do not establish changes of mind. Speculation about ASH does not confirm a scheduled full-data presentation.
