## Abstract

LINKER-AL2 studies linvoseltamab safety and hematologic complete response in relapsed/refractory systemic light-chain amyloidosis; second-line BCMA bispecific positioning is one clinician’s proposal. The report cites 10 physicians, including five medical oncologists and one hematology-oncology physician, and includes speakers described in the sources reviewed for this edition as presenting the trial; questions also concern meaningful organ recovery and serious infections. What organ-recovery, durability, safety and sequencing evidence would justify routine second-line use beyond trials or individual access? The sources place LINKER-AL2’s initial Phase I dose-escalation results at ASCO 2026.

## Open Medical Affairs question

What additional organ-recovery, durability, safety and sequencing evidence would justify routine second-line positioning rather than trial-based or individually accessed use?

Raised by the leading finding. The report shows what clinicians said and the sources behind it; it does not settle the question.

## Findings (titles)

- F1. A consequential second-line positioning proposal comes from one clinician, not an established consensus
- F2. Favorable tolerability assessments must travel with the serious infection and severe adverse-event observations
- F3. Rapid light-chain suppression drives enthusiasm, while meaningful organ recovery remains an explicitly open question for some authors
- F4. The strongest comparative language is experiential or mechanistic, not evidence of comparative efficacy
- F5. Retreatment timing involves both avoiding delay and avoiding unnecessary treatment toxicity
- F6. Eligibility and work-up shape who can act on the encouraging findings
- F7. Bispecific-versus-CAR-T choice includes urgency, repeated administration and preservation of later trial options
- F8. Could off-the-shelf delivery extend access beyond specialist centers?
- F9. What must Phase II establish, and what constrains AL-specific development?

## What sits behind each finding

Counts describe what the report retained, not how many clinicians hold a view. A statement can support more than one finding.

## Sample findings

Paraphrased; the full report links each statement to its source.

### F3. Rapid light-chain suppression drives enthusiasm, while meaningful organ recovery remains an explicitly open question for some authors

A hematology-oncology physician on a podcast addressed how the timing of hematologic responses relates to organ-response timing in LINKER-AL2.

Source: podcast, 2026-09-03.

### F9. What must Phase II establish, and what constrains AL-specific development?

The report distinguishes a physician’s question on X about future development from results in the LINKER-AL2 study population.

## Scope and limits

This report does not judge efficacy, estimate HCP prevalence or consensus or give treatment advice.

- Discussion that does not name the trial may be missing from this edition.
- Not yet captured in this edition: a remark on fatal treatment-emergent adverse events during treatment for relapsed or refractory systemic light chain amyloidosis.
- Not yet captured in this edition: a view on eligibility, treatment burden and recruitment access for investigational alternatives in relapsed or refractory light-chain amyloidosis.
- Not yet captured in this edition: a view on access to investigational cellular therapy as an alternative for relapsed or refractory AL amyloidosis.
- Not yet captured in this edition: a view on access constraints and care-team assessment for alternative investigational treatment in relapsed or refractory AL amyloidosis.
- Some passages in the sources used in this report were not fully assessed for clinician views, so a view there may be missing from this edition.
- 127 statements in the sources reviewed for this edition are about other patients or topics, or only mention the trial, and are not counted.
- Further source limits: discussion that describes the trial without naming it may be incomplete; some discussion that describes the trial without naming it may be missing; sources about other trials are not included.
- The sources reviewed for this edition are not a representative sample of HCP opinion. Reposts, repeated summaries and accounts of the same dataset should not be counted as independent clinical confirmation.
- Some potentially relevant social posts and a webinar passage were not fully evaluated, including safety commentary. The report therefore may not capture every consequential reported harm or qualification.
- Information about alternatives is incomplete: NEXICART-2's study phase and recruiting locations are not fully described, and eligibility, treatment schedules and practical access for ABBV-383 and remantamig are incompletely covered. This limits comparisons of trial suitability and access.
- Discussion that does not name the study or uses different wording may be missing.
- No exact-trial results publication, protocol document or exact linvoseltamab label was available for this edition. Source-reported response figures, follow-up, administration descriptions and safety details could not be fully reconciled against those documents.
- The early treatment appraisals concern a small cohort and exclude the most advanced cardiac involvement; they cannot establish safety or benefit for all patients with relapsed/refractory AL amyloidosis.
- Some podcast and webinar passages cannot be assigned to a named speaker and remain unnamed. A participant list does not show who said those words.
- Some speakers' identities or clinical roles are not established in the sources used in this report; each statement is attributed only as far as its source shows.
- Enrollment locations and availability are historical descriptions from the relevant webinars, not assurances of current access. Planned locations should not be assumed to have been enrolling. Publication dates do not establish session dates or changes in opinion.
