## Abstract

MagnetisMM-6 asks whether elranatamab with daratumumab and lenalidomide improves disease control over a daratumumab-based quadruplet in transplant-ineligible patients with newly diagnosed multiple myeloma. Clinicians in the sources reviewed for this edition question comparator relevance, and some make treatment attractiveness conditional on duration. Early activity draws enthusiasm in their commentary, while prolonged treatment draws scrutiny. Durability, comparative benefit and the possibility of finite treatment remain open.

## Open Medical Affairs question

What finite-course evidence would make the regimen attractive without replacing an efficacy advantage with an unacceptable risk of earlier relapse?

Raised by the leading finding. The report shows what clinicians said and the sources behind it; it does not settle the question.

## Findings (titles)

- F1. Finite treatment is a condition of attractiveness for some clinicians, not an established option
- F2. Comparator relevance became an explicit clinical question
- F3. Manageable acute toxicity does not settle the cumulative infection question
- F4. Early activity generates substantial optimism, but durability and comparative benefit remain separate questions
- F5. MRD is discussed as a route to personalization, not a ready-made stopping rule
- F6. Patient selection includes reasons to withhold intensity, not only reasons to add treatment
- F7. Sequencing and replacement strategies remain conditional research questions
- F8. Monthly dosing is not the same as a low-burden or universally outpatient program

## What sits behind each finding

Counts describe what the report retained, not how many clinicians hold a view. A statement can support more than one finding.

## Sample findings

Paraphrased; the full report links each statement to its source.

### F2. Comparator relevance became an explicit clinical question

An X discussion asked whether Dara-VRd rather than Dara-Rd should be the phase 3 comparator for the elranatamab combination.

Source: X thread, 2025-09-17.

### F4. Early activity generates substantial optimism, but durability and comparative benefit remain separate questions

An X post requested MRD findings and infection data with further follow-up of the elranatamab triplet.

The discussion includes posts on both dates; the date of the individual cited post is not stated.

## Scope and limits

This report does not judge efficacy, estimate HCP prevalence or consensus or give treatment advice.

- Discussion that does not name the trial may be missing from this edition.
- For 2 recordings, a transcript separated by speaker was not available, so quotes come from the original automatic transcript and may lack speaker labels.
- 1 cited commentary passage leaves a source question open, such as who was speaking. The report attributes no further than the source shows.
- Some clinician views in the sources used in this report are not yet captured as evidence in this edition; the next refresh takes them up.
- Some passages in the sources used in this report were not fully assessed for clinician views, so a view there may be missing from this edition.
- Out of scope: 6 passages in the reviewed sources discuss other trials or topics outside this report's scope; they are not part of this edition.
- 422 statements in the sources reviewed for this edition discuss other treatment settings or topics unrelated to this trial's question and are not included.
- Further source limits apply, so some matching discussion may be missing.
- The sources reviewed for this edition do not represent all HCPs or establish how common any opinion is. Reposts, organizational reporting and multiple media versions of the same discussion should not be counted as independent clinician agreement.
- Relevant discussions that do not name the trial or use different terminology may be missing. One social discussion was inaccessible, and portions of several long recordings were not fully evaluated, so less prominent views may be absent.
- Some recordings do not identify who voiced the relevant passage. Those views remain unnamed even when a program lists recognizable participants.
- No full protocol or trial-results publication was available to reconcile the reported efficacy, safety, denominator and follow-up differences. Reported numbers are what the cited speakers or organizations said.
- Earlier descriptions of a DRd comparator and later descriptions of DVRd are preserved in their own temporal context. Publication dates do not establish the date of a design decision or a change in an individual's opinion.
- Favorable response, tolerability and biological-rationale comments do not establish randomized comparative benefit, validated MRD-guided stopping or a preferred finite treatment duration.
- Class-wide experience, individual patient experience, other frontline regimen choices and sequencing hypotheses are distinguished from observations made specifically in MagnetisMM-6. Proposed alternatives to current induction remain research questions, not established treatment replacements.
- Raffaele Di Giacomo's clinical role is not stated in the sources reviewed; the author's questions should not be treated as an additional confirmed HCP endorsement.
