## Abstract

TALENTOP asks whether adding liver resection, followed by atezolizumab–bevacizumab, improves outcomes over continued systemic treatment after initial disease control in liver-confined HCC with macrovascular invasion. Commentary comes from X, LinkedIn and Bluesky posts, podcasts and YouTube presentations. The discussion turns on whether conversion surgery warrants a practice change while overall survival remains unresolved, whether stable disease is sufficient for surgical selection—a point of disagreement—and whether resection adds patient-centered benefit over maintenance.

## Open Medical Affairs question

Which responding patients have sufficient expected incremental benefit to justify surgery, and what evidence would make that discussion credible across settings?

Raised by the leading finding. The report shows what clinicians said and the sources behind it; it does not settle the question.

## Findings (titles)

- F1. Practice-change enthusiasm does not establish an overall-survival benefit
- F2. Upfront surgical harm is central, and safety descriptions are not interchangeable
- F3. Two selection gates—and disagreement about stable disease—limit applicability
- F4. Response should prompt multidisciplinary reassessment, not automatic continuation
- F5. Well-tolerated systemic treatment can weaken the patient’s reason to choose surgery
- F6. Does conversion surgery add patient benefit over continued systemic treatment?
- F7. Integrated-treatment enthusiasm includes alternatives and speculative cross-study comparisons

## What sits behind each finding

Counts describe what the report retained, not how many clinicians hold a view. A statement can support more than one finding.

## Sample findings

Paraphrased; the full report links each statement to its source.

### F3. Two selection gates—and disagreement about stable disease—limit applicability

A LinkedIn post addresses who qualifies for TALENTOP after atezolizumab–bevacizumab and how to assess applicability across HCC populations.

### F6. Does conversion surgery add patient benefit over continued systemic treatment?

A YouTube presentation asks whether resection after a systemic treatment response adds survival or quality-of-life benefit compared with continuing medication.

Who voiced this question is not established in the sources reviewed for this edition.

This historical treatment discussion does not report comparative TALENTOP outcomes.

## Scope and limits

This report does not judge efficacy, estimate HCP prevalence or consensus or give treatment advice.

- Discussion that does not name the trial may be missing from this edition.
- Not yet captured in this edition: a view on continued systemic treatment burdens when conversion surgery is not indicated or tolerated.
- Not yet captured in this edition: a view on conversion-treatment alternatives to resection, followed by consolidation and possible treatment cessation.
- Not yet captured in this edition: a view on short follow-up, tentative recurrence-free survival extrapolation, and randomized confirmation of benefit from conversion surgery.
- Not yet captured in this edition: a view on postoperative recurrence-free survival versus response duration as a proposed benchmark for surgery or continued systemic treatment.
- Not yet captured in this edition: a view on benefits and risks of conversion surgery versus maintenance therapy for hepatocellular carcinoma with vascular invasion.
- Not yet captured in this edition: a view on whether increasingly tolerable systemic treatment limits the additional quality-of-life benefit of conversion resection.
- Not yet captured in this edition: a view on the safety trade-offs of conversion surgery versus maintenance systemic therapy after initial treatment.
- Some passages in the sources used in this report were not fully assessed for clinician views, so a view there may be missing from this edition.
- 30 statements in the sources reviewed for this edition are about other patients or topics, or only mention the trial, and are not counted.
- Further source limits: discussion that describes the trial without naming it may be incomplete; some discussion that describes the trial without naming it may be missing; sources about other trials are not included.
- This report describes the sources reviewed for this edition, not all HCP discussion or a representative sample of clinical opinion.
- Parts of the early conversion-treatment discussion remain incompletely represented, including treatment-stopping burdens, nonoperative local options and comparisons of benefit duration.
- Repeated posts, reposts, relayed presentations and organizational accounts cannot be treated as independent clinician endorsements or used to estimate consensus.
- The identity of speakers in the 2021 conversion-treatment presentation and the POST ESMO 2025 episode is not established for the statements used here.
- Some speakers' identities or clinical roles are not established in the sources used in this report; each statement is attributed only as far as its source shows.
- Differences in endpoint labels, safety percentages, denominators and emphatic survival interpretations remain attributed to their authors rather than reconciled into a single clinical result.
- Historical patient experiences and preliminary conversion-treatment observations are not TALENTOP comparative outcomes.
- No separate protocol document or exact combination-product label was available; the registry and published results provide the primary study context.
